Every combine happens in a Oven (or a portable Spit), found near the tradeskill merchants in most towns. Your Intelligence or Wisdom (whichever is higher) governs your chance to gain a skill point, and a failed combine keeps the container but loses the ingredients, so buy your materials in bulk and expect some waste on the way up.
