Every combine happens in a Fletching Kit, found near the tradeskill merchants in most towns. Your Dexterity, Intelligence, or Wisdom (highest of the three) governs your chance to gain a skill point, and a failed combine keeps the container but loses the ingredients, so buy your materials in bulk and expect some waste on the way up.
